Student Health Services covers 80% of the reasonable and customary charges for medically necessary durable medical equipment. Students are responsible for a 20% copayment. Replacements/repairs are not covered. Student Health Services, or Forest Park Pediatrics, LC in the case of covered children, will assist in evaluating the need for artificial appliances, such as eye glasses, hearing aids, braces, orthotics, or biofeedback programs, but coverage for the device is not provided. Pediatric oral care is provided separately under the included dental health plan.
