Student Health Services will provide all Center for Disease Control (CDC) Advisory Committee for Immunization Practices (ACIP) adult recommended vaccines free of charge if the Covered Adult meets the eligibility criteria. All other vaccines will be provided at cost. These services are limited to Student Health Service and Forest Park Pediatrics Providers only. Immunization are not covered by any other provider.
Pediatric immunizations are provided by Forest Park Pediatrics. Pediatric travel immunizations are provided by Forest Park Pediatrics. Student Health Services will provide all Center for Disease Control (CDC) Advisory Committee for Immunization Practices (ACIP) recommended vaccines free of charge from birth to age 18 if the Covered child meets the eligibility criteria. All other vaccines will be provided at cost.
For any other vaccine, Students and Covered Dependents will be responsible for an 80% coinsurance requirement and Student Health Services will cover 20% of the charges and you will be responsible for the remaining cost.
Student Health Services also will provide students with exit statements of health for internships (but you must schedule them with Student Health Services) and with immunization letters, if needed. The first exit statement of health for internships will be provided at no charge. Any additional copies/requests will be provided for a service fee.
Dependent children are not eligible to utilize Student Health Services and instead should seek immunizations from the PPCP.
