As part of the Program of Animal Care and Use, the PHS Policy and the Guide state that each institution must establish and maintain an Occupational Health and Safety Program (OHSP) encompassing all personnel who have contact with animals.
As part of the WashU program, an annual completion of the following questionnaire is required:
