Freedom PPO Plan
The Freedom PPO plan offers one of the largest PPO network out of all dental carriers, expanding the in-network prepaid providers from 36 to 180 in a 5 mile radius. The Freedom PPO plan also offers you a large in-network provider list while out of town with no changes in your coverage. To assist you with a locating an Assurant provider a free secure benefit tools app is available to receive provider information on the go. This app will provide quick access to find a network dentist nearby. The university will continue to subsidize fully the prepaid plan or if you elect to enroll in the Freedom PPO, the university will partly subsidize the Freedom PPO plan reducing your monthly responsibility to $13.20. Please review your options the enrollment is only offered once a year.
